Summary of safety incidents in FRNY reported from January 2016 to date

Hello Everyone, Please find attached a summary of the safety incidents and near misses that were reported to me in the 2016 calendar year (January 1 - July6, 2016). I hope that sharing this information with you will prevent similar situations from happening in the future. The second document attached is a more detailed description of the incident that occurred on May 20, 2016, which led to the Purdue Emergency Responders coming to the building, evacuating the building and restricting access to one of our labs, FRNY 3140, until air quality testing confirmed no presence of H2S, CO and VOCs in the air. This document was written by the safety officers and glove box super users in Dr. Agrawal's group and I would like to thank them for sharing the details of this incident with us. Please pay attention to the "Lessons learned" part; there is very valuable information in that section. Please continue to report all safety incidents and near misses using the form "Safety Incident/Near Miss Report" posted on our website: https://engineering.purdue.edu/ChE/aboutus/safety under "Workplace Injury Information". As a reminder, when someone is injured during a safety incident, even if the injury does not require medical attention, it still needs to be reported to REM, using the First Report of Injury (FROI) form: https://www.purdue.edu/ehps/rem/home/forms/froi.pdf. The form, which is also available on our website, needs to be submitted by the supervisor. Outside of windows cleaned

FYI - There is a crew working off a lift that is cleaning the building windows on the outside. At times they will have an exit door blocked when working above that area. Center for Healthy Living - Diabetes program open for enrollment 8/22/16 to 11/14/16

The Center for Healthy Living on Purdue's West Lafayette campus introduces its comprehensive "Living with Diabetes" program. This is free to benefits-eligible faculty and staff - including those who have opted out of Purdue medical coverage - and for spouses/same-sex domestic partners and adult dependents covered on a Purdue medical plan. Also open to parents and guardians of children under the age of 18 (covered on a Purdue medical plan) who have been diagnosed with Type 1 or Type 2 diabetes who would like to attend on behalf of their child(ren). The same plan provisions in regards to medication discounts would apply to the child(ren). Since that time, enrollment has been popular and participants have successfully continued to make changes to improve their lives. During the fall 2014 session and spring 2015 session, a total of 117 individuals successfully completed the program with an average 0.5 percent reduced A1C -- a measurement of a person's average levels of blood glucose, or blood sugar, over the past three months -- at baseline for type 2 diabetes. In our spring 2015 class, three individuals diagnosed with type 2 diabetes dropped their A1C by an average of 3.9 percent each by the end of the 12-week session. Spots still remain for the upcoming program. To enroll, call the center at 49-45505 by July 11. The basics * The program provides education and support for those who are living with diabetes. * Those covered on a Purdue medical plan will get discounts on their diabetes medications throughout 2016 and ongoing when they maintain participation in the program. * The program is available at no charge to benefits-eligible faculty and staff at all Purdue campuses - including those who have opted out of Purdue medical coverage - and for spouses/same-sex domestic partners and adult dependents covered on a Purdue medical plan. In addition, parents of minor children (covered on a Purdue medical plan) may participate on behalf of the child.
