Hello everyone, Please be advised that the mechanism of reporting work related injuries has changed, and the "First Report of Injury" (FROI) form is now completed and sent directly through DocuSign. The FROI form must be completed when a work-related injury occurs on the Purdue University campus. All injuries, regardless of how slight, must be reported to your supervisor immediately, but no longer than 24 hours after the injury. Upon notification of the injury, the supervisor will need to complete and submit the FROI via DocuSign, no later than 24 hours from when the injury was reported to them. Please follow the instructions on the DocuSign page and send me a copy of the document once submission is confirmed.
