Good morning, This is a gentle reminder that the completed ISP self-audit forms are due this coming Monday, February 5; see the initial email below. Please remember that ALL offices and labs/storage rooms need to be audited. Multiple offices can be included on the same form, but each lab/storage space needs a form. The forms need to be signed by the person responsible for that room, i.e.: PI, but can be completed by their designee ( e.g.: safety officer). Please return completed self-audit forms as PDF files via email to Gabriela Nagy (nagyg@purdue.edu<mailto:nagyg@purdue.edu>) by the end of day on Monday, February 5, 2024. Should you have any questions, please contact me. Thank you, Gabriela on behalf of the ChE Safety Committee __________________ Gabriela Nagy, PhD Industrial Education Director Davidson School of Chemical Engineering Room FRNY G051 Purdue University Tel: (765)-496-1710 E-mail: nagyg@purdue.edu<mailto:nagyg@purdue.edu> From: Nagy, Gabriela <nagyg@purdue.edu> Sent: Monday, January 22, 2024 11:05 AM To: 'chme@ecn.purdue.edu' <chme@ecn.purdue.edu>; 'chmegrad@ecn.purdue.edu' <chmegrad@ecn.purdue.edu> Cc: Corti, David S <dscorti@purdue.edu>; Kim, Sangtae <kim55@purdue.edu>; Nagy, Gabriela <nagyg@purdue.edu>; CHE Safety <chesafety@purdue.edu> Subject: Start of self-audits of ChE labs and offices! Importance: High Hello Everyone, Our School is scheduled to have the Integrated Safety Plan (ISP) review at the beginning of March 2024. Part of this process is the completion of the safety self-audit of ALL ChE laboratories AND offices. The Integrated Safety Plan Self-Audit Checklist<https://www.purdue.edu/ehps/rem/documents/forms/ispcheck.pdf> (also attached) is available online on the ChE School's website, and on EHS's website at: https://www.purdue.edu/ehps/rem/documents/forms/ispcheck.pdf. Please only use these forms, as they are the most up-to-date forms used University wide. One form needs to be completed for EACH lab/storage area/machine shop. However, multiple offices (but not labs) can be included on the same form. 1. The first eight sections have universal questions that apply to every area. For self-audits of office space, only section 1-8 have to be completed. I attach the Hazard Assessment Certification for FRNY offices for your information; please answer "yes" to questions 8.1 - 8.4 and "no" to question 8.7 for offices . 2. Sections 9 to 29 have questions related to specialized operations; research laboratories and shops should address all sections (section 1 through 29). 3. All self-audit checklists must be signed by PI/supervisor, but another person, the supervisor/PI's designee can complete the self-audit. 4. Electronic signatures are accepted on ISP self-audit forms. Please correct identified deficiencies, and record the corrections in the "Comments" section on last page of the form before submitting the forms. You should also record the deficiencies that were not corrected, and list the steps taken to correct them. In addition to completing the self-audit for your labs/shop, please review and update (to be dated within the last 6 months) the following: * Laboratory Door Posting<https://www.purdue.edu/ehps/rem/documents/forms/doorpost.pdf> - using this new form which includes a date stamp). This needs to be posted outside the lab, at every entrance including the connecting doors in between labs (if applicable) * Hazard Assessment Certification (should be placed inside the lab, near the entrance) * Chemical inventory for the lab (qualitative) * Lab Specific Chemical Hygiene Plan (at a minimum, the first page needs to be signed by the PI of the lab and dated within the last 6 months). * SDS Records (update printed SDS book with SDSs for newly purchased chemicals). If your group is keeping SDSs saved electronically, please make sure you follow the recommendations issued by the safety committee, that are listed HERE<https://engineering.purdue.edu/ChE/aboutus/safety/documents/Guidelines%20for%20storage%20of%20electronic%20copies%20of%20SDSs%20%2811-2016%29.pdf>. SDSs on record should be dated within the last 5 years. * Eye wash station flushing log - please flush eye-wash stations weekly and record the completion in a log. I attach a template that can be used for this activity. The ChE Safety Committee will perform safety inspections of all laboratories and offices at the beginning of February and will send the inspection reports to faculty and their group safety officers. Deficiencies identified in your audits and during the safety inspections will need to be addressed before the recertification meeting with EHS. The ChE Safety Committee (list of members posted on https://engineering.purdue.edu/ChE/aboutus/safety) is available to help. Please return completed self-audit forms as PDF files via email to Gabriela Nagy (nagyg@purdue.edu<mailto:nagyg@purdue.edu>) by the end of day on Monday, February 5, 2024.
