Hello Everyone, As the end of the academic year approaches, I would like to update you on the safety incidents/accidents we had in our School in this academic year. 2012-2013 was a "safety incidents free" year, but in 2013-2014 we had 4 reported incidents. Please find attached a document highlighting a summary of the incidents and the measures taken in order to prevent such unpleasant experiences in the future. Should an accident or incident occur in your lab/office, please let me know as soon as it happens, so we can work together in completing all required steps and report the incident to REM to comply with Purdue regulations. I also need to keep a copy of the incident report on file. All accidents or incidents need to be reported to REM using the "First Report of Injury" (FROI) Form located on REM's website: https://www.purdue.edu/rem/home/forms/froi.pdf. You can find the instructions on how fill in the FROI form at the following link: http://www.purdue.edu/rem/injury/froi.htm. The supervisor will also need to fill in the "Supervisor's accident investigation form" (https://www.purdue.edu/rem/home/forms/saif.pdf ); this is meant to identify the root cause of the accident, so that a relevant corrective action can be taken/implemented. In addition, I would like to ask you to bring to my attention the near-misses you experience in your lab or office. I would like to develop a database that could be used as a safety resource in preventing a near miss become an accident. In the future, I plan on sharing these safety incidents with you as soon as I have all the information needed, rather than waiting to compile all of them in an "End of Year Report".
