Please be sure you’ve completed a confidential MSE Emergency Contact form, including your contact info and at least one person OUTSIDE of the school or Purdue.   This form is kept confidential within the MSE department and used only in urgent emergency situations.  We would also need this information in case you are ever involved in an emergency and we need to contact someone on your behalf.

Please complete and return to me asap, either electronically or printed hard copy.  If you have updated information since your last contact form was completed, please also complete a new form.
