I’ve attached the most recent version which includes checkboxes for other lab guided training: LN2, Comp Gases, Sharps, Haz Waste & Biological Safety Review. First Aid Kit review was removed as we do not maintain First Aid Kits in any of our labs.
I use this checklist to document new lab user training and as a guideline for REM requirements.

Comments are welcomed but the document is working well so far.
