I2D-list Potential partnership with BPHC
Hi All, Shah Lab has been making concerted efforts in understanding the opportunities to utilize our supported solutions and technologies to help alleviate challenges at home, applying the concepts of reciprocal innovation. And like with every project, here too, we have been talking to like-minded entities that work in local communities. One of such entity is Bronx Partners for Healthy Communities.<http://www.bronxphc.org/> (BPHC) BPHC, works with communities in Bronx and is leading an initiative called DSRIP (Delivery System Reform Incentive Payment) , which aims to reduce avoidable emergency room (ER) and hospital admissions by 25% by building a provider system of coordinated, community-based care that focuses on treatment, wellness and disease prevention. DSRIP Program is a major collective effort to transform New York’s Medicaid Healthcare Delivery System from a fragmented system, overly focused on inpatient care, to an integrated and community based system focused on providing care in or close to home. BPHC leads this work in partnership with more than 230 local organizations. BPHC has shared with us some of the key areas that they have seen as challenges in their work (please see the attached excel sheet). These are very high level challenges/program summaries and before we dig deeper to clarify areas where we could potentially be helpful, I wanted to ask you, if there are any areas, where you can see your research being applicable? Please let me know your thoughts/inputs/questions by end of this week (Friday, 23rd).
