Please find attached a summary of the safety incidents and near misses reported to me from December 2021 to date. I hope that sharing this information with you will prevent similar situations from happening in the future.

Please continue to report all safety incidents and near misses using the form Safety Incident/Near Miss Report posted on our website: https://engineering.purdue.edu/ChE/aboutus/safety under “Workplace Injury Information”.

As a reminder, when someone is injured during a safety incident, even if the injury does not require medical attention, this still needs to be reported to REM, using the First Report of Injury (FROI) portal: https://www.purdue.edu/ehps/rem/froi/ai.html. The form needs to be submitted by the supervisor. Please also add me (nagyg@purdue.edu) on the list of recipients.
