Hello Everyone,

 

Our School is scheduled to have Integrated Safety Plan (ISP) review during the month of February 2021; this usually takes place in August every year, but it was delayed in 2020 due to the Covid-19 pandemic. Part of this process is the completion of the safety self-audit of all ChE laboratories and offices.  With many faculty, staff and graduate students continuing to work remotely this year, the self-audit of offices is only needed for those offices that are physically used at this time; we don’t want anyone to come to campus only to complete the self-audit of their office, but if you come on campus anyway, please conduct the self-audit of your office space.

 

The ISP Self-Audit checklist can be found on the ChE School's website (https://engineering.purdue.edu/ChE/aboutus/safety), under the "Laboratory Safety Requirements" heading, or on REM's website: https://www.purdue.edu/ehps/rem/isp/index.html . Please only use these forms, as they are the most up-to-date forms that are now used at University level. I also attach the most updated form, for your convenience. One form needs to be completed for each lab/storage area/machine shop. However, multiple offices can be included on the same form.

 

  1. The first eight sections have universal questions that apply to every area. For self-audits of office space, only section 1-8 have to be completed. I attach the Hazard Assessment Certification for FRNY offices for your information; please answer “yes” to questions 6.1 – 6.4 for offices.
  2. Sections 9 to 27 have questions related to specialized operations; research laboratories and shops should address all sections (section 1 through 27).
  3. All self-audit checklists must be signed by PI/supervisor, not necessarily by the person completing the self-audit.
  4. Electronic signatures are accepted on ISP self-audit forms.

 

In addition to completing the self-audit for your labs/shop, please review and update the door posting, hazard assessment (should be placed inside the lab, near the entrance), chemical inventory (Form HCP4), and lab specific Chemical Hygiene Plan (at a minimum, the first page needs to be signed by the PI of the lab and dated within the last 6 months). They should be all accurate and dated within the last six months. If your group is keeping SDSs saved electronically, please make sure you follow the recommendations issued by the safety committee, that are listed HERE.

 

Please return completed self-audit forms to Gabriela Nagy via email (nagyg@purdue.edu) by the end of day on Monday, February 1, 2021. Electronic submission of completed and signed forms is preferred. However, if you prefer to submit printed forms, you can slide them under my door in FRNY G051 (and email me to let me know), or hand them to me in person if I am in my office.
