Hello Everyone,
Our School is scheduled to have the Integrated Safety Plan (ISP) review at the beginning of April 2026. Part of this process is the completion of the
safety self-audit of ALL ChE laboratories AND offices using the
Integrated Safety Plan Self-Audit Checklist (also attached). Please
only use this form -it was updated in Dec. 2025 and is the current form used University wide.
One form must be completed for EACH office/lab/storage area/machine shop, and the
completed forms must be signed on the last page by both the person completing the self-audit and the PI/supervisor (electronic signatures are accepted).
The first page of the document provides more information – please pay close attention to the lower half of the page.
Some additional information/guidance:
- The
first 11 (eleven) sections have universal questions that apply to all areas.
For self-audits of office space, only sections 1-11 should be completed. I attach the
Hazard Assessment Certification (HAC) for FRNY offices for your information –
this shows that no PPE is required when working on an office. The document is stored in my office and posted in the main office; you do not have do print and display the HAC in your office.
- For
Section 6:
- Section 6.1. and 6.2. – the answer should be “Yes” for both. (Lab specific HAC are displayed in labs and for offices you can prompt individuals to the main office to consult
the HAC).
- If you answered “yes” to section 6.3 (more applicable to labs), the answer to section 6.4. should also be “yes” ( Lab specific HAC are displayed in labs)
- Section 7.1:
For offices, the answer should be “No”
- Section 8:
For offices, the answer should be “N/A” for all sections 8.1-8.8.
- Section 9:
- Section 9.3:
For labs: the answer should be “yes”. Please flush the eyewash station and log the flushing. If your lab does not have a log, please use the attached template and post it close to the eye wash.
- Section 9.4:
For labs: please answer “Yes”. This is something new and Jason will be completing this task once EHS is providing the necessary kit.
- Section 10:
For offices: the answer should be “yes” for sections 10.1-10.4 and “No” for section 10.5 (see the attached HAC for offices)
- Section 11 must be completed for offices (answer “Yes” to section 11.1-11.3 for offices)
- Sections 12-32 have questions related to specialized operations;
research laboratories and shops should address all sections (section 1 through 32).
Please correct identified deficiencies and record the corrections in the table on page 9; you can also add comments in the ”Comments” section on last page.
In addition to completing the self-audit for your labs/shop,
please review and update (to be dated within the last 6 months) the following:
- Laboratory
Door Posting - use this form which includes a date stamp. This needs to be posted outside the lab,
at every entrance including the connecting doors in between labs (if applicable),
- Hazard Assessment Certification (should be placed inside the lab, near each entrance, including connecting doors),
- Chemical inventory for the lab (qualitative) – if stored in electronic form please provide a QR code to the updated chemical
inventory,
- Lab Specific Chemical Hygiene Plan (at a minimum, the first page needs to be signed by the PI of the lab and dated within
the last 6 months),
- SDS Records
(update printed SDS book with SDSs for newly purchased chemicals). If your group is keeping SDSs saved electronically, please make sure you follow the recommendations issued by the safety committee, that are listed
HERE.
SDSs on record should be dated within the last 5 years. You can also provide a QR code to the collection of lab’s SDSs,
- Eye wash station flushing log –
please flush eye-wash stations weekly and record the completion in a log (use the attached template).
The ChE Safety Committee will perform
safety inspections of all laboratories and offices at the end of February and will send the inspection reports to faculty and their group safety officers. Deficiencies identified in your audits and during the safety inspections must be addressed before
the recertification meeting with EHS. The ChE Safety Committee (list of members posted on
https://engineering.purdue.edu/ChE/aboutus/safety) is available to help.
