Lessons learned (summary of ChE safety incidents July-October) and other safety communication
Hello everyone, Please find attached a summary of the safety incidents and near misses reported to me from July 2022 to date. I hope that sharing this information with you will prevent similar situations from happening in the future. Please continue to report ALL safety incidents and near misses using the form Safety Incident/Near Miss Report<https://engineering.purdue.edu/ChE/aboutus/safety/documents/safety-incident-report-form-template.docx> posted on our website: https://engineering.purdue.edu/ChE/aboutus/safety under "Workplace Injury Information". Recommendation: The ChE safety committee recommends that all labs using gas cylinders keep a dedicated cylinder wrench, such as the Matterson TW-5 Cylinder Handwheel Wrench in the lab, and use it in case the cylinder cap is stuck on and can't be easily removed by hand. This tool can also be used to open cylinder valves. I attach some information regarding the use of a gas cylinder wrench and more information can be found at this link: https://store.mathesongas.com/tw-series-cylinder-wrenches/ Eyewash stations in the lab need to be flushed weekly, and the completion of this task needs to be recorded/logged. Please find attached a template of a log that can be used to record the completion of this task. Please keep the log somewhere near the eye wash station as the safety committee (and REM) is checking on this during safety inspections. Any other logs are accepted, but documentation that the task was completed on a weekly basis is required. The Annual Safety Chair Meeting is scheduled for November 9, 2022, from 8 am- 11 am in Stewart Center, Room 214. Room 218 will feature a safety fair from 7:30 to 11:30 a.m. with a variety of vendor displays along with REM and public safety representatives to answer questions about safety needs. The Safety Chair Meeting is split into three sessions, with the third and final session, 10:15-11 a.m., having a laboratory theme targeting an academic research audience.
