I hope that sharing this information with you will prevent similar situations from happening. Please continue to report all safety incidents and near misses using the form "Safety Incident/Near Miss Report" posted on our website: https://engineering.purdue.edu/ChE/AboutUs/Safety.html under "Workplace Injury Information". As a reminder, when someone is injured during a safety incident, even if the injury does not require medical attention, this needs to be reported to REM, using the First Report of Injury (FROI) form: https://www.purdue.edu/ehps/rem/home/forms/froi.pdf.
