Lessons learned - Recent safety incidents in FRNY
Hello Everyone, Please find attached a document describing two recent safety incidents that occurred in our building in the last few days, and were reported to me yesterday. I hope that sharing this information with you will prevent similar situations from happening in the future. Please continue to report all safety incidents and near misses using the form "Safety Incident/Near Miss Report" posted on our website: https://engineering.purdue.edu/ChE/aboutus/safety under "Workplace Injury Information". As a reminder, when someone is injured during a safety incident, even if the injury does not require medical attention, this still needs to be reported to REM, using the First Report of Injury (FROI) form: https://www.purdue.edu/ehps/rem/documents/forms/froi.pdf. The form, which is also available on our website, needs to be submitted by the supervisor. Please also add me (nagyg@purdue.edu<mailto:nagyg@purdue.edu>) on the list of recipients, when you submit the completed FROI form by email. Should you have any questions, please let me know.
