Hello Everyone,
Please find attached a summary of the safety incidents and near misses that were reported to me in the 2016 calendar year (January 1 – July6, 2016). I hope that sharing this information with you will prevent similar situations from happening
in the future.
The second document attached is a more detailed description of the incident that occurred on May 20, 2016, which led to the Purdue Emergency Responders coming to the building, evacuating the building and restricting access to one of our
labs, FRNY 3140, until air quality testing confirmed no presence of H2S, CO and VOCs in the air. This document was written by the safety officers and glove box super users in Dr. Agrawal’s group and I would like to thank them for sharing the details of this
incident with us. Please pay attention to the “Lessons learned” part; there is very valuable information in that section.

As a reminder, when someone is injured during a safety incident, even if the injury does not require medical attention, it still needs to be reported to REM, using the First Report of Injury (FROI) form:

by the supervisor. Please also add me (nagyg@purdue.edu) on the list of recipients, when you submit the completed FROI form by email.
