Abstract:

Blood hemoglobin (Hgb) testing is one of the most common clinical laboratory tests and has been extensively performed for numerous patient care needs. However, even point-of-care blood Hgb measurements require invasive blood sampling, potentially causing iatrogenic blood loss when repeated tests are needed. The performance of current noninvasive blood Hgb tests is often limited and incomparable to clinical laboratory blood Hgb tests. To overcome these limitations, we have developed a mobile health (mHealth) technology for noninvasive blood Hgb quantification. As an easily accessible sensing site, the inner eyelid is used because of the relatively uniform microvasculature and the absence of melanocytes. Spectral learning and color accuracy (color correction) algorithms are used for reliable blood Hgb assessments. Spectral learning allows mapping from a sparse space (RGB values) to a dense space (multiple wavelengths). In other words, spectral learning virtually transforms the smartphone camera into a spectroscopic imaging device. A tissue-specific color accuracy method extracts true color information of the inner eyelid, independent of smartphone models and ambient light conditions during photo acquisition. An easy-to-use color reference chart with a unique color gamut is designed to be highly sensitive to blood Hgb and peripheral perfusion, permitting single-shot photo acquisition. The spectral learning and color accuracy algorithms combined with spectroscopic blood Hgb modeling enable noninvasive, real-time, and accurate blood Hgb measurements over a wide range of blood Hgb values. Our mHealth blood Hgb testing can be scalable, robust, and sustainable both in resource-limited and homecare settings, facilitating the tempo of clinical translation.
